Consent To Disclosure of
Personal Information Form
This form acts as formal
confirmation that you are authorizing 3CI and the partner law
enforcement agency, to collect personal information about you in
order to conduct a Criminal Record Check. It authorizes 3CI and the
partner law enforcement agency to search for the following
information in the National Repository of Criminal Records, based on
the name(s) and date of birth.
- Records of criminal
convictions for which a pardon has not been granted, and
conditional and absolute discharges which have not been removed
from the CPIC system in accordance with the Criminal Records Act
- Wanted person
information, which is information relating to a person who is
arrestable and/or for whom a warrant has been issued.
- Accused person
information, which is information relating to a person against
whom criminal legal proceedings have been commenced and are in
progress.
- Prohibitions and other
judicial orders which are in effect.
All
information is collected and Disclosed according to Section 29(1)
& 32 of the MFIPPA.

Vulnerable Sector Screen
Form

confirmation that you are authorizing 3CI and the partner law
enforcement agency, to collect personal information about you in
order to conduct a Vulnerable Sector Screening. A vulnerable sector
screening may provide the following information from the Canadian
Police Information Centre:
- Criminal record (adult,
including summary offences)
- Findings of not guilty
by reason of mental disorder
- Probation, prohibition
and other judicial orders which are in effect
- Pardoned Sexual
Offences, listed in the schedule to the Criminal Records Act
- Apprehension by Police,
under the Mental Health Act

Authority to Release
Information Form
The Authority to Release
Information document will clearly make the applicant aware you will
be conducting background checks and provides protection to you
should the individual wish to contest the fact that you asked
questions. Whether you conduct pre-employment background checks or
not, it is advisable to have every individual you are considering
for any position to sign an 'Authority to Release Information'
form. It is our experience that a company should have all applicants
submit a signed copy of the Release at the application stage of
recruiting. Should you choose to wait, it is ideal to have this
Release signed prior to pre-screening / resume review.

User Agreement Form
The
User Agreement Form, provides formal confirmation that 3CI is
authorized to act as your Criminal Record Check Agent. This document
ensures only approved users from your organization are authorized to
conduct background checks and that all information provided by 3CI
will be held in strict confidence and kept confidential, except to
the extent that law requires disclosure to others.
